   Proposal Description:
   Subjective memory complaints (SMCS also now known as Subjective Cognitive Concerns) are proposed to define a hypothetical Stage 3 of preclinical Alzheimer disease, where individuals are cognitively normal but have biomarker evidence of elevated cerebral amyloid burden, biomarker evidence of neuronal injury, and subtle cognitive deficits (generally operationalized as SMCs).  The   hypothesis in the this study is that informant-reported SMCs better predict neuropathologic AD compared with self-reported SMCs, because informant reports are more likely to identify cognitive change.                                     SA1.  Determine the frequency of SMCs in the baseline UDS assessment of cognitively normal individuals (here operationalized as CDR 0 at baseline assessment).  Self-reported SMCs are defined by a                                 individual to Q10 of Form B6 (Geriatric Depression Scale) and/or a response to Q1 in Form B10 (Clinician Judgment of Symptoms), whereas an informant report of SMC in the individual is defined by a                         in Form B9.                                                                      SA2.  Compare the predictive value of self-reported vs informant-reported SMCs for correlation with the presence of neuropathologic AD, as defined by NIA-Reagan criteria.                                                              SA3.  Evaluate the contribution of other variables (interval from date of baseline UDS assessment to date of death age at baseline APOE4 status gender education MMSE score at baseline relationship of informant to individual Q9 on Form A2) total score on Form B6 (GDS) # of meds (from Form       A4) race).
